2.12.20 - “We will also add more extra-mural Liaison Nurses to support the transfer of seniors out of hospital and back to their homes or Special Care Homes. Their role is to coordinate with extra-mural so that when a patient is discharged, they perform a full assessment of what medical equipment may be needed in the home, along with any other support they may need while they recover.”

The polimeter considers this promise fulfilled since at least one nurse has been newly hired to participate in the program.
“Health Seniors Pilot Project (HSPP) is moving forward with the Nurse Practitioner to support the R & R Program. A Project Site Coordinator was hired and it is going well in the Saint John area. Working to inform EMP and the Hospital about the project. They have also been able to recruit a Nurse Practitioner for the project and start Rehab and Reablement (R & R) rounds to five EMP units. The purpose of this project is to better support the patients through the R & R program by having a dedicated primary care provider”

There does not appear to have been any real progress in the recruitment of nurse practitioners affiliated with the New Brunswick’s extra-mural program.
Regarding Enhancing Role of NB with R&R Patients in the South. Ginette Pellerin indicated EM is struggling with recruitment for this project and if qualified personnel cannot be hired in the South, EM will look at implement this pilot project in a different region
